Personal data in this document is collected only to prepare and file the association's conveyance-related application and supporting project/title record, communicate about that purpose, verify expressly supplied information, complete the property transaction or filing, and retain an evidentiary record.

The record will be held by __________ and may be shown only to the buyer, seller, authorised broker, advocate, bank, registering authority, revenue authority, society/developer, tax authority or other person directly involved in the stated transaction.
