Punjab: Article 4 (Affidavit), Schedule 1-A to the Indian Stamp Act, 1899 as applicable to Punjab — non-judicial stamp paper, commonly a small fixed denomination, bought from a licensed stamp vendor or e-stamped through the Stock Holding Corporation of India Limited (SHCIL, shcilestamp.com), the sole e-stamping agency for Punjab. Chandigarh: Article 4, Schedule I to the Indian Stamp Act, 1899 as extended to the Union Territory, the same nominal head, e-stamped via SHCIL or bought at the Treasury, Sector 17. Both articles have been revised by amendment in recent years and neither department publishes a single dated rate card for a simple affidavit; confirm the current denomination with the stamp vendor or SHCIL before the notary appointment — a notary will not swear an affidavit onto paper bought afterwards.

Not a registrable instrument. Section 17 of the Registration Act, 1908 reaches instruments affecting immovable property and a handful of other named categories; this affidavit is neither, so there is no Sub-Registrar step. It is complete on notarisation and is then produced directly before UIDAI, the Passport Seva Kendra, a bank, an employer or any other record-holder that needs the name updated, or annexed to a separate Gazette Name Change Application where that is additionally required.

Sworn before a Notary Public appointed under the Notaries Act, 1952 and the Notaries Rules, 1956, or affirmed before an Oath Commissioner appointed by the District & Sessions Judge of the district. Both are equally acceptable to the authorities this affidavit is typically produced before. The deponent signs in the notary's or Oath Commissioner's presence, not before.

None required by law. A notarised or sworn affidavit is authenticated by the notary's or Oath Commissioner's own seal and register entry, not by attesting witnesses. Some notaries nonetheless ask for an identifying witness where the deponent is not personally known to them, particularly where the identity proof produced is still in the former name — carry a second photo ID in case one is asked for.
