The Passport Seva portal or your Regional Passport Office has asked for a standard-format affidavit alongside a fresh, reissue or change-of-particulars passport application. Annexure D is commonly asked for as a general affidavit verifying a particular the application cannot otherwise support with a standard document. Annexure F is commonly asked for where the application involves a change of name following marriage or divorce. Annexure I is commonly asked for where it involves a change in appearance or signature since the last passport was issued.
The Passport Seva instruction booklet and your Regional Passport Office are the authority on which annexure a given applicant needs and exactly what it must say, and the letter-to-purpose mapping has shifted across editions of that booklet over the years.
