Personal data in this document is collected only to process and evidence the landlord's limited consent for broadband installation, communicate about that purpose, verify the information expressly supplied, and retain an evidentiary record.

The record will be held by __________ and may be shown only to the landlord, tenant, authorised property manager or property consultant involved in this tenancy, the utility/service provider, society or building manager directly involved in the stated purpose, and a statutory authority where disclosure is required for the transaction recorded here.
