CONSENT AND DECLARATIONS OF A PERSON TO BE APPOINTED A DIRECTOR
I, __________, son, daughter or spouse of __________, residing at __________, DIN __________, PAN __________, state as follows.
2. NOT DISQUALIFIED. To the best of my knowledge I am not disqualified from being appointed a director under the Companies Act, 2013. In particular, I am of sound mind, not an undischarged insolvent, not convicted by a court of an offence involving moral turpitude and sentenced to imprisonment, and have not been disqualified by a court or the Registrar, and no company of which I am or was a director has failed to file its accounts or annual returns or repay deposits or debentures in the manner that would disqualify me.
(If this line is blank, I hold no other such post.)
5. UNDERTAKING. I shall tell the Company at once in writing if anything stated above changes, including any disqualification, and I shall keep the Company's confidential information confidential, act honestly and in the Company's interest, and give any further disclosure the law requires at the times it requires.
6. The statements above are true to my knowledge and belief and nothing material has been concealed. I understand that the Company will rely on them, and that the prescribed forms for consent and declarations, where the law requires them, will be signed separately.
