Registration is done through the Tourism Department's online e-Services Portal, and this sworn affidavit is the declaration you attach to that application, confirming your ownership, residence and compliance with the Rules' conditions. Not the registration application itself, and not a substitute for it — file this alongside the online form, not instead of it.
