We, the undersigned, being not less than seven persons associated for the purpose described in the accompanying Memorandum of Association, apply for registration of a society under the Societies Registration Act, 1860, under the proposed name "__________". We enclose the Memorandum of Association (Part B), the Rules and Regulations (Part C), the verification affidavit, and the other documents listed below.

PART C — RULES AND REGULATIONS
- MEMBERSHIP
6.1 Eligibility: __________.
6.4 Membership ceases on resignation, on a member ceasing to meet the eligibility described above, or on expulsion by the General Body for conduct prejudicial to the Society's objects, by the process its Managing Committee lays down consistently with natural justice.
- GENERAL BODY MEETINGS
7.1 The Annual General Meeting is held __________.
7.2 An Extraordinary General Meeting may be called by the Managing Committee, or on a written requisition of not less than one-fifth of the members, on fourteen days' notice.
7.3 Quorum for a General Body meeting: __________. If quorum is not present, the meeting stands adjourned to a date the Managing Committee fixes, at which the members present, whatever their number, constitute quorum.
- THE GOVERNING BODY (MANAGING COMMITTEE)
8.1 The first governing body is as stated in clause 4 above. Its term is __________ years, after which fresh elections or appointments shall be held in accordance with a schedule the Managing Committee notifies to members in advance.
8.2 The Managing Committee manages the day-to-day affairs of the Society, consistently with these Rules and Regulations and with decisions of the General Body, and may fill a casual vacancy in its own number by co-option until the next election.
8.3 Under s.3A of the Societies Registration Act, 1860, a list of the persons constituting the Managing Committee, as elected or appointed at each Annual General Meeting, shall be filed with the Registrar within fourteen days of that meeting.
- FUNDS, BANKING AND AUDIT
9.1 The funds of the Society consist of membership fees, subscriptions, donations and any other lawful receipts, and shall be applied only towards the objects of the Society.
9.2 The Society's bank account shall be operated by __________.
9.3 The accounts of the Society shall be __________, and the audited accounts shall be placed before the Annual General Meeting.
- AMENDMENT
10.1 The Rules and Regulations may be amended __________.
10.2 A change of the Society's name or objects requires the special majority s.12 or s.12A of the Societies Registration Act, 1860 prescribes, and takes effect only on the Registrar's approval under that section.
- DISSOLUTION
11.1 The Society may be dissolved by a resolution of not less than three-fifths of its members, consistently with s.13 of the Societies Registration Act, 1860.
11.2 On dissolution, after satisfying the Society's debts and liabilities, the remaining funds and property shall not be distributed among the members in any manner, but shall be given or transferred to some other society with objects similar to those of this Society, to be determined by the members at or before the time of dissolution, consistently with s.14 of that Act, or by the civil court if the members are unable to agree.
