AFFIDAVIT IN SUPPORT OF AN APPLICATION FOR A RETAIL DRUG LICENCE (CHEMIST SHOP)
I, Ramesh Chander Verma, son, daughter or wife of Late Sh. Om Prakash Verma, aged 47 years, residing at House No. 211, Sector 15-A, Chandigarh 160015, do solemnly affirm and state on oath as follows.
- 1. That I am applying for a licence for retail sale of drugs other than those in the schedules that need special conditions at the shop called Verma Medical and General Store, situated at Shop No. 14, Sector 22-D Market, Chandigarh 160022, and the applicant for the licence is the deponent, as sole proprietor.
- 2. That I hold the premises as the owner, and have the legal right to carry on the business of selling medicines there. The shop has an area of about 180 square feet, is clean, well lit and ventilated, and suitable for storing and selling medicines. A refrigerator is installed in the shop for that purpose.
- 3. That the sale of drugs in the shop will always be carried on under the personal supervision of Neha Kapoor, a registered pharmacist (Registration no. 12345, Punjab State Pharmacy Council), who has agreed to be the competent person in charge, and that I shall tell the licensing authority at once if he or she leaves, and shall not sell drugs in the shop until another registered pharmacist takes charge and the authority has been told.
- 4. That I shall buy drugs only from licensed dealers against proper bills, keep the records of purchase and sale, the prescription register and the stock the law requires, sell prescription drugs only on a valid prescription of a registered medical practitioner, store drugs as their labels require, not sell expired, spurious or banned drugs, and allow the Drugs Inspector to inspect the shop and its records at any time.
- 5. That neither I nor, to my knowledge, any partner or director of the applicant has been convicted of an offence under the drug laws, or has had a drug licence cancelled or refused, or is otherwise disqualified from holding a licence, and that no other licence of this kind is held in the shop.
- 6. That I shall follow the Drugs and Cosmetics Act and Rules and the conditions of the licence, shall tell the licensing authority of any change in the premises, the pharmacist or the persons who run the business, and shall renew the licence before it ends.
VERIFICATION. Verified at Chandigarh on 1 April 2026 that the contents of this affidavit are true and correct to my knowledge and belief, that nothing material has been concealed, and that no part of it is false.
