- 2. The disputed entries. __________ The total I dispute is __________.
- 3. Why I dispute them. __________ __________
- 4. What I have done. __________ __________
- 5. What I require. Please: (a) register this as a dispute and give me its reference number; (b) investigate it, including by raising it with the merchant or the card network where that is the right step; (c) hold the disputed amount, and any interest and late fee that would arise only from it, out of my payable balance until you have decided, and not treat the card as in default for that amount meanwhile; (d) if the dispute is upheld, reverse the amount and any interest and fees charged on it; and (e) tell me your decision in writing, with the reasons, and the name of the officer handling this.
- 6. If you do not. I will pay the part of the statement that I do not dispute. If I do not receive a satisfactory written reply in the time your card terms and the Reserve Bank's directions allow, I will take the complaint to the Reserve Bank's Ombudsman and, if needed, to the District Consumer Commission. This letter does not waive any of my rights.
ENCLOSURES
__________
LEGAL BASIS
Punjab and Chandigarh: The Reserve Bank of India's directions on credit card issuers require them to have a way for a card holder to dispute a charge and to deal with it fairly, and the card terms usually set a time within which a dispute must be raised. A card issuer's service to its card holder is a service under the Consumer Protection Act, 2019, so a complaint of deficiency can be taken to the District Commission, and the Reserve Bank's Integrated Ombudsman Scheme in force allows a complaint to the Ombudsman after the issuer rejects the dispute or does not reply. No fee is payable on this letter.
PERSONAL DATA NOTICE
This notice is given under the Digital Personal Data Protection Act, 2023.

The record will be held by __________ and may be shared with the card issuer and its grievance officers, the merchant or card network if the dispute is taken up with them, the Reserve Bank's Ombudsman, the sender's advocate, and any Commission or court that hears the matter.
If the matter does not proceed, the personal data will be retained for 90 days and then erased. If the matter proceeds, the record will be retained for the transaction or authority matter plus 12 months, except where a statute requires a longer named retention period.
A person named in this document may request correction or erasure where the purpose has ended, withdraw consent for future consent-based processing, or raise a grievance by writing to __________. The record-holder should acknowledge and reply within 30 days.
