Answer what you can. The draft beside these boxes is rewritten as you go, so you can see exactly what each answer does to the document.

What you type stays in this browser. Nothing is sent anywhere until you choose to keep a copy — and closing this tab loses it.

Nothing is amiss in what you have written. The document is simply not finished, and the draft marks each gap where an answer will go.

A calendar date — the day, the month and the year. Dates in a document like this are read against one another, so a date typed here can change what another date is allowed to be.

For example: 16 November 2026

For example: Ludhiana

How will you send this?Needed

For example: The Nodal Officer, Credit Cards Division, Example Bank Ltd

At what address is it sent?Needed

An email address somebody actually reads. Notices under a document like this can be sent to it, so a working one matters more than a tidy one.

For example: cardsdisputes@examplebank.example.com

For example: Gurpreet Singh Dhillon

What is your address?Needed

An email address somebody actually reads. Notices under a document like this can be sent to it, so a working one matters more than a tidy one.

For example: gurpreet.dhillon@example.com

A number somebody can be reached on. Ten digits is the usual Indian form, and a country code in front of it is fine if you want it there.

For example: +91 98150 47731

For example: Dispute of a double charge of 12,400 rupees on credit card ending 8820

For example: my credit card ending 8820

A calendar date — the day, the month and the year. Dates in a document like this are read against one another, so a date typed here can change what another date is allowed to be.

For example: 5 November 2026

For each: the date, the merchant as shown in the statement, and the amount.

A long answer. Several lines are fine, and a break you make between paragraphs is the break the document keeps.

For example: 24 October 2026, Sharma Electronics, Ludhiana, 12,400 rupees, charged twice (two entries of the same amount on the same date).

An amount in rupees. Commas and a ₹ sign are both fine — write it the way you would write it anywhere else, and the document is given the tidy form of it.

For example: 12400

Why do you dispute it?Needed

The facts in order: what you bought or paid, what you were told, and what the statement shows.

A long answer. Several lines are fine, and a break you make between paragraphs is the break the document keeps.

For example: I bought a television from the merchant on 24 October 2026 for 12,400 rupees and swiped my card once. The slip shows one payment, but the statement shows two debits of the same amount.

Each attempt: the date, how, what was said; write "I have not been able to contact the merchant." if so.

A long answer. Several lines are fine, and a break you make between paragraphs is the break the document keeps.

For example: I visited the shop on 28 October 2026 and again on 2 November 2026. The owner said it was the bank's problem.

The date, how, and any reference number; write "None so far." if none.

A long answer. Several lines are fine, and a break you make between paragraphs is the break the document keeps.

For example: I raised a dispute on the helpline on 6 November 2026, reference 3390177, and was told to write.

Number each paper. Send copies and keep the originals.

A long answer. Several lines are fine, and a break you make between paragraphs is the break the document keeps.

For example: 1. Statement dated 5 November 2026 with the entries marked. 2. Copy of the merchant's bill and the card slip. 3. Messages with the merchant.

For example: Kuldip Singh Sandhu

An email address somebody actually reads. Notices under a document like this can be sent to it, so a working one matters more than a tidy one.

For example: kuldip.sandhu@example.com

The live draft

The draft, as it stands

This is the whole document, not a sample of it. It is rewritten every time you change an answer.

A long blank line — __________ — stands wherever an answer has not been given. It is left that visible on purpose, so an unfinished document cannot be mistaken for a finished one.

No answer has been given, so every place a detail belongs is standing blank. The shape of the document is worth reading even now.

  1. 5. What I require. Please: (a) register this as a dispute and give me its reference number; (b) investigate it, including by raising it with the merchant or the card network where that is the right step; (c) hold the disputed amount, and any interest and late fee that would arise only from it, out of my payable balance until you have decided, and not treat the card as in default for that amount meanwhile; (d) if the dispute is upheld, reverse the amount and any interest and fees charged on it; and (e) tell me your decision in writing, with the reasons, and the name of the officer handling this.
  1. 6. If you do not. I will pay the part of the statement that I do not dispute. This letter does not waive any of my rights.

ENCLOSURES

__________

LEGAL BASIS

Punjab and Chandigarh: The Reserve Bank of India's directions on credit card issuers require them to have a way for a card holder to dispute a charge and to deal with it fairly, and the card terms usually set a time within which a dispute must be raised. A card issuer's service to its card holder is a service under the Consumer Protection Act, 2019, so a complaint of deficiency can be taken to the District Commission, and the Reserve Bank's Integrated Ombudsman Scheme in force allows a complaint to the Ombudsman after the issuer rejects the dispute or does not reply. No fee is payable on this letter.

PERSONAL DATA NOTICE

This notice is given under the Digital Personal Data Protection Act, 2023.

The purpose of collecting the personal data in this document is to identify the card holder, record the charge disputed and the reasons, and let the card issuer investigate and reply.

The record will be held by __________ and may be shared with the card issuer and its grievance officers, the merchant or card network if the dispute is taken up with them, the Reserve Bank's Ombudsman, the sender's advocate, and any Commission or court that hears the matter.

If the matter does not proceed, the personal data will be retained for 90 days and then erased. If the matter proceeds, the record will be retained for the transaction or authority matter plus 12 months, except where a statute requires a longer named retention period.

A person named in this document may request correction or erasure where the purpose has ended, withdraw consent for future consent-based processing, or raise a grievance by writing to __________. The record-holder should acknowledge and reply within 30 days.

Data minimisation: this document does not collect a full Aadhaar number, an Aadhaar-card copy, a password or bank credentials.

Personal-data instruction: __________.

Reading costs nothing. Keeping a copy needs an account.

The opening of the draft is on this page as you answer it. Once every answer is in, the button below opens the whole draft, watermarked — still without an account. Printing it, or keeping a clean copy, is where an account is asked for. That is the whole arrangement.

A kept copy has to belong to somebody, and that is the reason for the account. There is nothing further to it.
