I, Sunita Sharma, S/o Ram Kishan Sharma, aged about 41 years, by occupation Service, resident of House No. 214, Sector 12, Sonipat, Haryana - 131001, holding Aadhaar bearing No. XXXX XXXX 4821, do hereby solemnly affirm and declare as under:
1. That I am the deponent named above. I am a citizen of India, I am of sound mind and competent to swear this affidavit, and the facts stated below are true to my own personal knowledge.
2. That this affidavit concerns my own name and identity — save that, instead, it concerns the name and identity of Aarav Sharma, a minor, my son, born on 1 April 2026, of whom I am the natural guardian, and on whose behalf I make this affidavit from facts within my personal knowledge.
3. That I am making this affidavit for the following purpose: To have the mutation (dakhil kharij) of Khasra No. 118/2, Village Kharkhoda, entered in my name, where the sale deed records me as Sunita Sharma but the jamabandi and my Aadhaar record me as Sunita Devi. This affidavit is being submitted to the office of the Tehsildar, Office of the Tehsildar, Kharkhoda, District Sonipat. It relates to Mutation Application No. 1476 / Khasra No. 118/2, Khewat No. 42, Village Kharkhoda.
4. That the name of the person described in paragraph 2 above has been recorded differently in different documents, in the following versions:
(a) "Sunita Sharma" — as appearing in: Aadhaar card, PAN card, Sale Deed dated 12.03.2021 registered as Vasika No. 3341.
(b) "Sunita Devi" — as appearing in: Jamabandi for the year 2018-19, Village Kharkhoda; ration card; electricity bill for Consumer No. 4402118.
(c) "S. Sharma" — as appearing in: School Leaving Certificate issued by Govt. Sr. Sec. School, Kharkhoda, dated 30.04.1999.
(d) "Sunita" — as appearing in: Bank passbook, Account No. 3092xxxx1104, Punjab National Bank, Kharkhoda branch.
Further, the name of a relative of mine, namely my father, has likewise been recorded differently in the same set of documents, namely as "Ram Kishan Sharma" and as "Ramkishan", appearing in: 'Ram Kishan Sharma' in my Aadhaar and PAN; 'Ramkishan' in the jamabandi and in my School Leaving Certificate. Every such version refers to one and the same person, that is to say my said relative, whose correct and complete name is "Ram Kishan Sharma".
5. That all the versions of the name set out in paragraph 4 above refer to one and the same person, that is to say the person described in paragraph 2 above, and to no other person whatsoever. There is no second individual answering to any of those versions in the documents referred to.
6. That the difference in the recorded name arose in the following manner: The jamabandi entry was made in Urdu and later transliterated into Hindi by the office, and my surname was written as 'Devi' instead of 'Sharma'. I have always used 'Sharma'.. In particular, "Sunita Rani Malik" is my maiden name, by which I was known before my marriage, which was solemnised on 2 April 2026, and "Sunita Sharma" is the name I have used since my marriage. My marriage is registered and the Marriage Certificate bearing MC/2009/04412 dated 18.11.2009, Registrar of Marriages, Sonipat is annexed to this affidavit.
7. That the correct and complete name, which I request may henceforth be treated as the correct name in the records of the authority to which this affidavit is given, is "Sunita Sharma". The said name has also been published as follows: Published in Dainik Jagran and The Tribune on 04.02.2024; Gazette of India Part IV dated 16.03.2024.
8. That the different versions of the name have not been used, and are not being used, for any fraudulent, dishonest or unlawful purpose. They have not been used to obtain any benefit, allotment, payment, loan, licence or record twice over, nor to conceal any identity, nor to defeat any claim of any person. The difference is only a difference in the way the name was written in the records, and nothing more. Except as stated in paragraph 4, the person described in paragraph 2 above is not known by any other name. No proceeding, civil or criminal, is pending in which the identity of that person, or the difference between these names, is in question.
