A sworn statement of your present marital status where some authority needs it on record — most often alongside a marriage registration application, a passport or visa application, a government job, admission or scholarship form, or an adoption proceeding. It covers three situations: never married at all; married and since divorced, without remarrying; or married and since widowed, without remarrying. This affidavit does not itself register a marriage, dissolve one, or prove a death — it is your own sworn statement, made on the strength of the decree or certificate it refers to, not a substitute for that decree or certificate. Attach a copy of the divorce decree or the death certificate where the receiving authority asks for one. Not for a joint declaration by both parties to an intended marriage under the Special Marriage Act, 1954 — that notice-of-intended-marriage procedure needs its own separate declaration in the form that Act prescribes, sworn by each party about age, status and the absence of a prohibited relationship, and is a different document from this one.

स्टाम्प ड्यूटी
A general affidavit, chargeable under Article 4 (Affidavit) of Schedule 1-A of the Indian Stamp Act, 1899 as applicable to Punjab, and under the corresponding entry of that Schedule as applicable to the Union Territory of Chandigarh. The commonly charged value for a general affidavit under that Article is a small fixed sum, typically Rs 10, printed on non-judicial stamp paper or produced as an e-stamp certificate through the Stock Holding Corporation of India Limited. Confirm the current value with the stamp vendor or on the e-stamping portal before buying it — Article 4 values are revised by state notification from time to time and are not always reflected promptly online.
रजिस्ट्री
Not compulsorily registrable. An affidavit of this kind does not create, limit or extinguish any interest in immovable property, so section 17 of the Registration Act, 1908 does not reach it. It is simply handed or submitted to whichever authority has asked for it — the Marriage Registrar, a passport office, an employer, an embassy — and kept on that authority's own file.
नोटरी
Must be sworn or affirmed before a Notary Public appointed under the Notaries Act, 1952, or before an Oath Commissioner appointed by the Punjab and Haryana High Court for the courts at the place of execution. Sign in the attesting officer's presence, not before — an affidavit signed beforehand and merely presented for stamping is not properly sworn and can be rejected by the authority it is submitted to.
गवाह
Not compulsory for the affidavit itself; a notary's or oath commissioner's attestation is what gives it evidentiary standing, not a lay witness's signature. The authority receiving it may separately ask for identifying witnesses, an apostille or consular attestation — a foreign embassy in particular often does — and that is a requirement of the receiving authority, not something this affidavit format fixes for you.
