This is an application addressed to a Station House Officer, not an instrument the Indian Stamp Act taxes — it transfers no right, creates no interest and is not a conveyance, agreement for consideration or power of attorney. No stamp paper or e-stamp is needed for it in either Punjab or Chandigarh, and a police station that asks you to buy stamp paper for a verification application is asking for something this form does not require.

Not applicable. This form is never presented to a Sub-Registrar and the Registration Act, 1908 has nothing to say about it — it does not touch immovable property and creates no registrable interest. What "registration" means here is entirely different: Punjab Police's own citizen portal and Sanjh Kendra network, and Chandigarh Police's citizen portal, both let you submit and track this application online, and the local police station keeps its own record of it. Submit it at, or address it to, the police station having jurisdiction over your residence or business premises in Punjab, or over your premises in the Union Territory of Chandigarh; the station typically forwards the employee's permanent-address particulars to the police station of that area for a local report, which is why the permanent address must be complete and accurate.

Not required. Attach self-attested photocopies of the identity proof named in this form and two recent passport-size photographs of the employee; carry originals for the police station to compare. Where the employer is a company, firm, RWA or society, attach a copy of the resolution or letter authorising the person who signs. Where the employee's own signature cannot practically be taken in person before submission, take it as soon as the employee is available and before relying on any verification obtained without it.

Not applicable in the sense a deed needs them — this is not executed inter partes and nobody attests it. What the form itself provides for instead is the employee's own consent, in the employee's own signature, to being verified, which is the safeguard this kind of document actually needs: a verification report obtained on particulars the employee never confirmed is weak evidence if their identity is later disputed. Keep the police station's acknowledgement or the online portal's reference number, and follow up for the verification report rather than assuming silence means a clear report.
