Sir or Madam,
- 1. The member. Harpreet Singh was a member of the provident fund through Sharma Textiles Pvt Ltd, Ludhiana. The universal account number is 100XXXXXXXXX. The provident fund account number is PB/LDH/0012345/000/0001234.
- 2. The death. Harpreet Singh died on 1 April 2026 at Chandigarh. The death certificate is DTH/2026/118, and a copy is enclosed.
- 3. What is claimed. The claim is for the provident fund balance of the member.
- 4. My right to claim. My relationship to the member: Spouse. I claim as the nominee recorded for the member's account.
- 5. The legal heirs. Apart from me, the other legal heirs and family members of Harpreet Singh are: Naina Singh and Meera Singh, Chandigarh.
- 6. The papers. I enclose the claim form(s) prescribed by your office, filled in and signed as the form requires, and the other papers listed below. Please check them on receipt and tell me in writing, at once, if anything is missing or wrongly filled, so that I can put it right.
- 7. Where to pay. Please pay the amount due to the account held by Aarav Singh with State Bank of India, Sector 35, Chandigarh, account ending 4821. I will give the full account number on your own form and not in this letter.
- 8. What I ask you to do.
(a) Acknowledge this letter and the papers in writing, and give me the claim reference number.
(b) Tell me the name and telephone number of the officer who will handle the claim.
(c) Tell me from time to time where the claim stands, and, when it is settled, the amount paid under each head and how it was worked out.
(d) If you reject any part of the claim or need more papers, tell me the reason in writing and the remedy open to me.
- 9. What I accept. I understand that a nominee is paid so that you are safely discharged, and that this does not by itself decide who owns the money as against the other legal heirs. If you pay me and it is later found that someone else had a better right, I will account to that person for what I received.
