Aadhaar Form 1: enrolment and update (resident Indian aged 18 and above)

Form 1 is UIDAI's Aadhaar enrolment and update form for a resident Indian, or a non-resident Indian with proof of address in India, aged 18 years and above. You use it at an Aadhaar enrolment centre, either to enrol for the first time or to update details. For an update you fill in only the information to be updated. Original documents are shown at the centre and returned. Children and other categories have their own forms.

Issued by: Unique Identification Authority of India (UIDAI)

The form itself is in English, exactly as the issuing office prints it.

How to fill it in

  1. Use capital (block) letters only. Items 1 and 2: tick Enrolment or Update, and Resident Indian or Non-Resident Indian.
  2. Item 3: name exactly as in your proof-of-identity document, leaving out titles such as Mr, Shri, Dr and aliases. Then gender, date of birth (or age; tick Verified, Declared or Approximate), email and mobile number.
  3. Item 4: tick the basis of your enrolment or update: Document verification, or Confirmation by Head of Family (HoF).
  4. Item 5 (document-based): fill the address only if you are enrolling or updating your address, with the post office and PIN code mandatory. Then write the document you present as proof of identity, proof of address and, optionally, proof of date of birth.
  5. Item 6 (only for Head of Family based enrolment or address update): the Head of Family's name and Aadhaar number, the relationship, the proof-of-relationship document, and the Head of Family's signature.
  6. Item 7 (only for an update): your Aadhaar number, and tick what is to be updated (biometrics, name, date of birth, gender, address, mobile, email, or the POI and POA documents).
  7. Read the declaration, then sign or put your thumb impression in the applicant box. The verifier at the centre signs and writes his or her name; leave that part for the centre.
