LPG Know Your Customer (KYC) form (Indian Oil, Bharat Petroleum, Hindustan Petroleum)
This is the common Know Your Customer (KYC) form of the three oil companies (version 3, 2014), as published on the Ministry of Petroleum and Natural Gas's PM Ujjwala Yojana website. You give it, with a copy of one proof of identity and one proof of address, to your LPG distributor. The distributor checks the copies against the originals. Your gas company may ask for its own current version, so confirm with your distributor. This is not a connection-transfer form.

How to fill it in
- Fill the form in black ink and block letters. Fields marked with a star are mandatory. Paste your photo in the box.
- 1 Personal details: your name exactly as on your photo ID, your gas consumer number if you already use LPG from these companies, your date of birth, and your father's or spouse's name (strike off the one that does not apply) and your mother's name.
- 2 Address and contact: the address for the LPG connection exactly as it appears in your proof of address, with PIN code, district and state, your mobile number and email. Write the proof-of-address category code from the list on the back (for example Aadhaar, driving licence, lease agreement, voter ID, utility bill or passport).
- 3 Other details: the proof-of-identity category code from the list and the card number, and the state and number of your ration card if you have one.
- Read the declaration, then sign with your name and the date. Leave the "To be filled by Dealer/Distributor" part and the tear-off acknowledgement for the distributor, and keep the acknowledgement when it is returned to you.
