Form 93: application for a new PAN (individual who is a citizen of India)
Form 93 is the application for allotment of a Permanent Account Number for an individual who is a citizen of India. Protean's PAN download page lists it for Indian citizens applying for a new PAN, to be submitted to a TIN facilitation centre, and its download page is titled Form 49A, the form this replaces. The form says you must give proof of identity, proof of address and proof of date of birth. Fill it in block letters in English.

The form itself is in English, exactly as the issuing office prints it.
How to fill it in
- Fill the form in English, in block letters, one character per box. Stick two recent colour photographs (4.5 cm x 3.5 cm, white background) as the form says, and sign across the left photograph.
- Items 1 to 4: your full name without abbreviations (1A) and your name exactly as on Aadhaar (1B), gender, date of birth and Aadhaar number.
- Items 5 to 10: residence address, office address, residential status (resident, non-resident or resident but not ordinarily resident), passport number (mandatory for the last two), tax number abroad if any, mobile number, email and landline.
- Part B, item 11: tick your source(s) of income (salary, business or profession, capital gains, other sources or no income).
- Part C, items 12 to 15: whether one parent is a single parent, your father's and mother's names, and which parent's name is to be printed on the PAN card.
- Part D (Assessing Officer code) and Part E (representative assessee): fill Part E only if someone applies on your behalf, for example for a minor. Part F: choose the address for communication.
- Part G: tick the proofs you attach (identity, address, date of birth). Write your name and capacity (self or representative assessee) in the declaration, then the place, date and your signature or left thumb impression in the box.
