Passport Annexure D: declaration by parents or guardian for a minor's passport

Annexure D is the specimen declaration, on plain paper, by the parent(s) or legal guardian of a minor child for whom a passport is being applied. Both parents sign it. It states that the child is an Indian citizen and that you take responsibility for the child's expenses. If you are a legal guardian, attach a copy of the court order appointing you. It goes with the child's passport application.

Issued by: Ministry of External Affairs (Passport Seva), under the Passports Act, 1967

The form itself is in English, exactly as the issuing office prints it.

How to fill it in

  1. Use plain paper. Write "I/We" and strike out the options (mother, father, parent(s), legal guardian(s)) that do not apply.
  2. Child's name and date of birth. Then the father's and the mother's name, citizenship, passport number, contact number, email and address.
  3. Paragraph 1: write the ARN (application reference number) of the child's passport application. Paragraphs 2 to 4 are the statements about citizenship, expenses and the consequences of false information: read them.
  4. Signatures: the father or legal guardian signs on the left and the mother or legal guardian on the right. Under each signature write the name and one ID number (passport number preferably, or Aadhaar, Voter ID, PAN or driving licence number), then the date and place.
  5. If a legal guardian signs, attach a copy of the court order appointing the guardian, as the note on the form says.
