Passport Annexure E: applicant's declaration on plain paper

Annexure E is the specimen declaration the applicant makes on a plain paper. It states the names of your parents and spouse, how long you have lived at your address, that you are an Indian citizen, and that no conviction, pending criminal case, warrant or summons stands against you. You fill it in yourself and sign it. Take it with your passport application when the passport office asks for it.

Issued by: Ministry of External Affairs (Passport Seva), under the Passports Act, 1967

The form itself is in English, exactly as the issuing office prints it.

How to fill it in

  1. Top: your name, "son/daughter/wife of Shri" with your father's or husband's name (strike out what does not apply), your address and your date of birth.
  2. Point 1: write your father's name, your mother's name and your wife's or husband's name.
  3. Point 2: write the date from which you have been living continuously at the address given above.
