Passport Annexure F: declaration for a new passport in place of a lost or damaged one
Annexure F is the specimen declaration an applicant makes to get a passport in place of a lost or damaged passport. You answer six questions: how and when it was lost or damaged and the FIR, whether you travelled on it, any government travel concessions, residence abroad, any objection by the passport authority, and any deportation at government expense. You sign it and take it with your passport application.
Issued by: Ministry of External Affairs (Passport Seva), under the Passports Act, 1967
The form itself is in English, exactly as the issuing office prints it.
How to fill it in
- Top: your name, "S/o, D/o, W/o Shri" with your father's or husband's name (strike out what does not apply) and your address.
- Question 1: how and when the passport was lost or damaged, when and at which police station the FIR was lodged, and how many passports you lost or damaged earlier.
- Question 2: whether you travelled on that passport; if yes, the flight number, the date and the port of entry into India.
- Question 3: whether you took any TR concession or foreign travel allowance, and the details if you did.
- Question 4: whether you are a non-resident Indian. If you live abroad, fill the table: country, length of residence (from and to) and the passport page numbers that carry the departure and arrival stamps.
- Questions 5 and 6: whether the passport authority ever raised an objection on the passport, and whether you were ever deported at government expense (and if so whether the cost was repaid). Then write the date and sign.
