Passport Annexure I: declaration by genetic parents for a child born through surrogacy

Annexure I is the declaration, on plain paper, by the genetic parent(s) who apply for the passport of a minor child born through surrogacy. It states that you are the genetic mother, father or parents, that the child has been in your exclusive physical custody since birth, and that you alone would defend any later complaint or case. It goes with the child's passport application.

Issued by: Ministry of External Affairs (Passport Seva), under the Passports Act, 1967

The form itself is in English, exactly as the issuing office prints it.

How to fill it in

  1. Use plain paper. Write "I/we" and the name(s) of the genetic parent(s) who are applying for the child's passport.
  2. Point 1: strike out the words that do not apply (mother, father or parents) and write the child's name.
  3. Points 2 and 3: write the child's name where the form asks. These points say the child has been in your exclusive physical custody since birth and that you alone would defend any complaint or case: read them before signing.
  4. Write the date and place, then sign with your address as the parent(s) or guardian(s) applying for the passport.
