Punjab: application for a new ration card or to add members to an existing one
This is the Punjab Food, Civil Supplies and Consumer Affairs department's application form for a new ration card, or for adding members to an existing ration card, under the National Food Security Act 2013 and the Smart Ration Card Scheme. The department's page also offers an online application. The form is for Punjab only; it is not a Chandigarh form. The head of family is a woman above 18 where there is one, otherwise the male head.

How to fill it in
- Top: tick Rural or Urban, write the district, village or town and Assembly Constituency, and tick whether you apply for a NEW RATION CARD or ADDITION OF MEMBERS IN EXISTING RATION CARD.
- Items 1 to 9: the head of family's name (a woman above 18; if there is none, the male head), date of birth, marital status, father's, spouse's and mother's names, the existing ration card number if you are adding members, mobile number and Aadhaar number.
- Items 10 to 23: category, the family's annual income, MGNREGA and eShram status, occupation, chronic illness or disability, agricultural land, education, and the other situations the form lists. Tick Yes or No on each line and give details where it asks.
- Items 24 to 29: electricity connection and latest bill details, any gas connection in a family member's name, type of house, rooms, and the complete present address.
- Items 30 to 32: type of family (single member or not), the number of additional family members (excluding the head), and anything else relevant. Paste a recent photo of the head of family and a recent group photo of the family.
- Read the self-declaration (no family member is a regular government employee; annual family income less than the amount printed on the form), then sign or put your thumb impression and date it.
- Next page: for each member to be added (other than the head), fill a block with name, sex, father's and mother's names, relationship with the head, marital status, date of birth, Aadhaar number, occupation, MGNREGA and eShram status, any illness or disability, and paste the member's recent photo.
