Form 6: application to be added to the electoral roll (new voter)

Form 6 is the Election Commission's application form for new voters. You use it to ask the Electoral Registration Officer to include your name in the electoral roll of your constituency, when you are applying for the first time and your name is not on any roll. You attach a self-attested copy of a document for your age and one for your address.

Issued by: Election Commission of India, under the Registration of Electors Rules, 1960

The form itself is in English, exactly as the issuing office prints it.

How to fill it in

  1. Top: write the number and name of your Assembly Constituency (or Parliamentary Constituency, only for Union Territories without a Legislative Assembly). Paste one recent passport-size colour photograph (4.5 cm x 3.5 cm, white background, unsigned) in the box.
  2. Items 1 and 2: your name in the State's official language and in English block letters, and the name of one relative (father, mother, husband, wife, or legal guardian in case of an orphan) in both.
  3. Items 3 to 6: mobile number and email (your own or the relative's), Aadhaar number (or tick that you do not have one), and gender.
  4. Item 7: date of birth, and tick one document as proof of date of birth (birth certificate, Aadhaar, PAN, driving licence, Class X or XII certificate, or Indian passport). If you have none, name another document.
  5. Item 8: your full ordinary-residence address, and tick one document as address proof (for example an electricity, water or gas bill, Aadhaar, bank passbook, passport, registered rent deed or sale deed).
  6. Items 9 and 10: disability details, only if applicable and optional; and, if a family member living with you is already on the roll at this address, that person's name, relationship and EPIC number.
  7. Declaration: write your place of birth and the month and year since when you have lived at the address in item 8, strike off whatever does not apply, then sign and date. Self-attest every copy you attach.
