Form 8: shifting of residence, correction of voter entries, replacement EPIC
Form 8 is the Election Commission's voter application form for four things: shifting your residence, correcting entries in the existing electoral roll, getting a replacement EPIC (voter card) without any correction, and marking a person with disability. You tick one of the four and fill only that part.
Issued by: Election Commission of India, under the Registration of Electors Rules, 1960
The form itself is in English, exactly as the issuing office prints it.
How to fill it in
- Top: write the number and name of your Assembly Constituency (or Parliamentary Constituency, only for Union Territories without a Legislative Assembly).
- Part I: your name, EPIC number, Aadhaar number (or tick that you do not have one), and your mobile number and email, or a relative's.
- Part II: tick one of the four: 1 Shifting of residence, 2 Correction of entries, 3 Replacement EPIC without correction, 4 Marking as Person with Disability. Fill only the matching section below.
- Section 1, shifting: write your present full address and attach a self-attested copy of one address proof (for example an electricity, water or gas bill, Aadhaar, bank passbook, passport, registered rent deed or sale deed). The form says you return your old EPIC.
- Section 2, correction: tick the entries to correct (name, gender, date of birth/age, relation type, relation name, address, mobile number, photo; at most 4), write the correct particulars, name the supporting document and attach a self-attested copy. Paste a photograph only if the photo is to be changed.
- Section 3, replacement EPIC: tick whether the card is lost, destroyed (flood, fire or other disaster) or mutilated. Return the mutilated or old card, or, if lost, attach a copy of the FIR or police report.
- Section 4, disability: tick the category (locomotive, visual, deaf and dumb, or describe another), the percentage, and whether a certificate is attached. Then read the declaration and sign with the date.
