Indemnity bond of applicant: duplicate allotment letter (CHB)
The applicant for a duplicate of a lost CHB letter, slip or certificate: it goes with the application.

How to fill it in
- Download the form and print it. Fill it in by hand, in clear block letters, with a blue or black pen.
- Fill in the blanks the form prints: Date of the bond, Name of the executant, Executant's S/o, D/o or W/o, House number and address, Duplicate copy of which document is required, Dwelling unit number and the others.
- Attach with it: The indemnity bond is on stamp paper of ₹15 or more. Self-attested copy of ID proof of both witnesses.
- Where it goes: Attach it to the application at the Reception Counter, Chandigarh Housing Board. It goes on stamp paper of ₹15 or more; the two witnesses sign it, and their ID proofs are attached.
- Official time limit: 30 working days, not counting the public notice (CHB Citizens' Charter, section 3.3).
- Sign where the form asks you to, check that no blank is left empty, and keep a photocopy for yourself.
