How to fill it in
- Download the form and print it. Fill it in by hand, in clear block letters, with a blue or black pen.
- Fill in the blanks the form prints: Name of the applicant, Father's / husband's name, Correspondence address, Mobile number, E-mail ID, Aadhaar number and the others.
- Attach with it: Affidavit of the applicant. Indemnity bond of the applicant. Three self-attested photographs of the applicant(s) along with the specimen signature. Self-attested copy of ID proof, Aadhaar card and PAN card of the applicant. Self-attested copy of ID proof of the witnesses of the indemnity bond. Receipts of processing charges (deposit online at the CHB website or at any HDFC Bank branch). Bank letter for permission to mortgage or lien marking.
- Where it goes: Reception Counter, Chandigarh Housing Board (addressed to the Secretary, CHB).
- Official time limit: 25 working days (CHB Citizens' Charter, section 3.8).
- Sign where the form asks you to, check that no blank is left empty, and keep a photocopy for yourself.
