How to fill it in
- Download the form and print it. Fill it in by hand, in clear block letters, with a blue or black pen.
- Fill in the blanks the form prints: Name of the applicant, Father's or husband's name, Correspondence address, Mobile number, E-mail ID, Aadhaar number and the others.
- Attach with it: Form-A (proforma for registration) and Form-B (proforma for the paying guest). Affidavit and indemnity bond of the applicant(s), as per the specimen. Self-attested copy of ID proof, Aadhaar card and PAN card of the applicant. Self-attested copy of ID proof of the witnesses of the indemnity bond. Self-attested copy of the allotment letter and physical possession slip. Self-attested copy of the transfer letter, if any. Self-attested copy of the No Due Certificate.
- Where it goes: Reception Counter, Chandigarh Housing Board (addressed to the Secretary, CHB).
- Sign where the form asks you to, check that no blank is left empty, and keep a photocopy for yourself.
