How to fill it in
- Download the form and print it. Fill it in by hand, in clear block letters, with a blue or black pen.
- Fill in the blanks the form prints: Place where the bond is made, Date of the bond, Your name, Your father's or husband's name, House number, Sector and the others.
- Attach with it: Self-attested copies of ID proof of the witnesses of the indemnity bond.
- Where it goes: Attached to the application at the CHB Reception Counter. Print on stamp paper of Rs.15 or more and attest before a Notary Public or Executive Magistrate, with two witnesses.
- Sign where the form asks you to, check that no blank is left empty, and keep a photocopy for yourself.
