How to fill it in
- Download the form and print it. Fill it in by hand, in clear block letters, with a blue or black pen.
- Fill in the blanks the form prints: Your name, Your father's or husband's name, Your address, Your age, Dwelling unit number, Sector and the others.
- Attach with it: CHB executes this agreement; the checklist names no stamp-paper value for it. Ask at the counter.
- Where it goes: Attached to the application at the CHB Reception Counter; the agreement is executed with CHB.
- Official time limit: 25 working days, excluding the public notice (Citizens' Charter s2.3, Annex A).
- Sign where the form asks you to, check that no blank is left empty, and keep a photocopy for yourself.
