Three photographs and specimen signatures sheet, intestate death (CHB)
Every applicant on a CHB application, who must attach this sheet with three self-attested photographs.

The form itself is in English, exactly as the issuing office prints it.
How to fill it in
- Download the form and print it. Fill it in by hand, in clear block letters, with a blue or black pen.
- Fill in the blanks the form prints: Name of applicant 1, Name of applicant 2, Name of applicant 3.
- Attach with it: Three self-attested photographs of each applicant, pasted in the photograph boxes. Sign by hand: partly on the photograph and partly on the sheet.
- Where it goes: Attach to the application and hand it in at the Reception Counter, Chandigarh Housing Board (addressed to the Secretary, CHB).
- Sign where the form asks you to, check that no blank is left empty, and keep a photocopy for yourself.
