How to fill it in
- Download the form and print it. Fill it in by hand, in clear block letters, with a blue or black pen.
- Fill in the blanks the form prints: Day of the month of signing, Month and year of signing, Name of the claimant, Your father's or husband's name, Your age, Your address and the others.
- Attach with it: CHB lists it as the "Agreement to be executed with CHB for lease rights"; it goes with the rest of the application checklist.
- Where it goes: Attached to the transfer application at the CHB Reception Counter. CHB lists it as the agreement to be executed with CHB for lease rights.
- Official time limit: 25 working days, excluding the public-notice period (CHB Citizens' Charter, section 2.3 and Annex A).
- Sign where the form asks you to, check that no blank is left empty, and keep a photocopy for yourself.
