Issued by: Greater Mohali Area Development Authority (GMADA), Government of Punjab

The form itself is in English, exactly as the issuing office prints it.

How to fill it in

  1. Download the form and print it on A4 paper. Fill in the blanks the form prints, in clear block letters, with a blue or black pen.
  2. The bank fills in the letter: the borrower's name and parentage, the borrower's address, the loan amount in figures and words, the property (type, number, sector, town), the purpose of the loan and the bank's name, then the amount it agrees to release. An authorised signatory of the bank signs it and writes the bank's name. Attach the completed letter to your application for permission to mortgage.
  3. Sign where the form asks you to, check that no blank is left empty, and keep a photocopy for yourself.
