How to fill it in
- Download the form and print it on A4 paper. Fill in the blanks the form prints, in clear block letters, with a blue or black pen.
- At the top write the plot or site number, the sector or phase and the size, and tick one: residential, commercial, institutional or industrial.
- Fill in lines 1 to 5: name of the allottee; name of the GPA / sub-GPA / special power of attorney holder (only if there is one); father's or husband's name; permanent address with telephone number; and correspondence address with telephone number.
- Line 6: the date the building plan was sanctioned. Line 7: tick whether the building is completely or partially constructed. Then sign, print your name in capital letters and write the date.
- Attach with it. The GMADA page for this service lists: a self-attested copy of the occupation certificate, a certificate from a plumber registered with GMADA (offered separately on this site), and a demand draft for road cutting charges if any. The page also notes an extra compounding fee for an unauthorised sewer connection.
- Official time limit stated on the GMADA page: 5 working days.
- Sign where the form asks you to, check that no blank is left empty, and keep a photocopy for yourself.
