Affidavit of legal heirs in a death case (HSVP)
Legal heirs of a deceased HSVP plot or house allottee who want the plot transferred into their names.

How to fill it in
- Download the form and print it. Fill it in by hand, in clear block letters, with a blue or black pen.
- Fill in the blanks the form prints, in order: the deponent's name; the name of the deceased and the relation to you; the plot number, sector and urban estate; the area in square metres.
- Write the date and place of death, then list every legal heir in the table: name, age and relation with the deceased.
- Write in the names of the heirs in whose favour the plot is to be transferred. The form states that the original death certificate is enclosed, so attach it.
- Write the place and date at the end and sign as the deponent(s). Have the affidavit signed and attested as the office requires, on the stamp paper it asks for.
- Sign where the form asks you to, check that no blank is left empty, and keep a photocopy for yourself.
