Form 2: application for a learner's or driving licence

Form 2 is the one application used for a new learner's licence, a new driving licence, adding a class of vehicle, renewal, a duplicate, or a change of address or name in the licence. You tick the service you want. The form has a physical-fitness self-declaration, and an annexure listing the documents to attach. Submit it at the Licensing Authority (the RTO), often online through Parivahan's Sarathi service. In Punjab and Chandigarh much of this is done online, and the printed form is uploaded or taken along.

How to fill it in

  1. At "The Licencing Authority", write the RTO you are applying to. Stick a passport-size photograph in the box (for an online application, upload a photo of not more than 50 KB, as the form says).
  2. "Services applying for": tick the service you want, for example new learner's licence, new driving licence, renewal, duplicate, or change of address or name.
  3. Item 1, "Class of Vehicles": for a new licence, tick the kind of vehicle, such as motor cycle without gear, motor cycle with gear, or light motor vehicle (a car).
  4. Item 2, "Personal details": in capital letters, write your Aadhaar number if you have one, your first, middle and last name, parent or spouse's name, date of birth, identification mark, gender, qualification, blood group, email (optional) and mobile number.
  5. Item 3: tick whether the name given is your father's, mother's, husband's or guardian's, and write it. Item 4, "Address": write your present address (it is printed on the licence) and your permanent address only if it is different. Enclose proof of address.
  6. Items 5 and 6: item 5 is only for adding a transport class, with the driving school's details. Item 6 is for an existing licence: write its number, classes, the office that issued it and its validity dates. Skip both for a first-time learner's licence.
  7. Item 7: tick the documents you are attaching from the annexure at the end of the form.
  8. "Declaration": say YES or NO to donating organs, state whether you have been disqualified from holding a licence, then sign with the date. Answer each question in the physical-fitness self-declaration (epilepsy, eyesight, limbs, night blindness and so on) honestly with Yes or No.
