How to fill it in
- Download the form and print it. Fill it in by hand, in clear block letters, with a blue or black pen.
- The form is a numbered list. Fill 1 to 4: date of birth, sex, the child's name, and the place of birth.
- Fill 5 to 8: the child's father's name, the child's grandfather's name, the child's mother's name, and the permanent address.
- Fill 9 to 11: the applicant's name, the applicant's address and mobile number, the applicant's signature, and the number of certificates applied for.
- Where it goes: ask at the Municipal Corporation office in Mohali which counter takes this form. The Corporation's website also lists some of these services as online applications, so ask which route it wants.
- Sign where the form asks you to, check that no blank is left empty, and keep a photocopy for yourself.
