How to fill it in
- Download the form and print it. Fill it in by hand, in clear block letters, with a blue or black pen.
- The form is a numbered list. Fill 1 to 4: date of death, sex, the name of the deceased, and the place of death.
- Fill 5 and 6: the deceased's father's or husband's name, and the permanent address of the deceased.
- Fill 7 to 9: the applicant's name, address and mobile number, the applicant's relation to the deceased, and the number of certificates applied for. Sign at the Applicant Signature line.
- Where it goes: ask at the Municipal Corporation office in Mohali which counter takes this form. The Corporation's website also lists some of these services as online applications, so ask which route it wants.
- Sign where the form asks you to, check that no blank is left empty, and keep a photocopy for yourself.
