New water connection application (MC Mohali)

How to fill it in

1. Download the form and print it. Fill it in by hand, in clear block letters, with a blue or black pen.
2. Rows 1 to 3: write the date of application, your name, and your father's or husband's name.
3. Row 4, address: city or village, locality and house number, district, telephone, phone or mobile number, and e-mail ID if you have one.
4. Row 5, water supply or sewer connection requirement: fill the lines for B.A. fees (if applicable), any other detail, plot or property number, and name of the scheme.
5. Row 6, documents to attach: the form lists a site plan of the plot, a self-declaration that the information is correct, a copy of ownership proof, a copy of the approved building plan, a plumber report, an allotment letter, a possession certificate and an NOC from the concerned department. Tick Yes in your column for each one you attach, and attach them. Leave the office column alone.
6. Row 7: sign as applicant. Leave the "For Office use" box and the acknowledgement slip for the office to fill in. Where it goes: ask at the Municipal Corporation office in Mohali which counter takes this form. The Corporation's website also lists some of these services as online applications, so ask which route it wants.
7. Sign where the form asks you to, check that no blank is left empty, and keep a photocopy for yourself.
