Addition of child's name in the birth record
Parents or grandparents whose child's birth is registered without a name.

How to fill it in
- Download the form and print it. Fill it in by hand, in clear block letters, with a blue or black pen.
- Fill in the blanks the form prints: Complete name of the child, Child's Aadhaar number, if any, Sex of the child, The child was born to, Mother's name, Mother's Aadhaar number, if any and the others.
- Attach with it: Application form and this name-addition form. Education certificate of the child (if available). Copy of passport. Aadhaar card of the parents and the child (if available).
- Where it goes: The Registrar, Births & Deaths, Municipal Corporation Chandigarh (the form is addressed to the Registrar). The portal does not name a counter.
- Official time limit: 10 days, as listed on the MC portal.
- Sign where the form asks you to, check that no blank is left empty, and keep a photocopy for yourself.
