How to fill it in
- Download the form and print it. Fill it in by hand, in clear block letters, with a blue or black pen.
- Fill in the blanks the form prints: Date of birth, Sex of the child, Name of the child, Name of the birth mother, if known, Name of the birth father, if known, Date and number of the adoption deed or order and the others.
- Where it goes: The department does not publish where this form is handed in. Ask at the Births & Deaths branch, MC Chandigarh.
- Sign where the form asks you to, check that no blank is left empty, and keep a photocopy for yourself.
