Parents or a relative who need copies of a birth already registered with MC Chandigarh.

How to fill it in
- Download the form and print it. Fill it in by hand, in clear block letters, with a blue or black pen.
- Fill in the blanks the form prints: Date of birth, Sex, Place of birth, Child's name, Father's name, Father's Aadhaar number, if any and the others.
- Attach with it: Your ID proof — the form asks for your name "with ID proof". Adding the child's name (item 12): an application on bond paper, as the form says.
- Where it goes: The Registrar, Births & Deaths, Municipal Corporation Chandigarh (the address printed on the form).
- Official time limit: Not printed on the form. Ask at the counter when you submit.
- Sign where the form asks you to, check that no blank is left empty, and keep a photocopy for yourself.
