Death certificate — application for copies

How to fill it in
- Download the form and print it. Fill it in by hand, in clear block letters, with a blue or black pen.
- Fill in the blanks the form prints: Date of death, Sex, Full name of the deceased, Deceased's UID number, if any, Place of death, Father's / husband's name of the deceased and the others.
- Where it goes: The Registrar, Births & Deaths, M.C. Chandigarh (the address printed on the form).
- Official time limit: 7 days, as listed in the department's online services portal for issue of a death certificate.
- Sign where the form asks you to, check that no blank is left empty, and keep a photocopy for yourself.
