How to fill it in
- Download the form and print it. Fill it in by hand, in clear block letters, with a blue or black pen.
- Fill in the blanks the form prints: Date of death, Deceased's first name, Deceased's middle name, Deceased's last name, Deceased's Aadhaar number, if available, Deceased's date of birth, if available and the others.
- Where it goes: The department does not publish where this form is handed in. Ask at the Births & Deaths branch, MC Chandigarh.
- Official time limit: 7 days, as listed in the department's online services portal for issue of a death certificate.
- Sign where the form asks you to, check that no blank is left empty, and keep a photocopy for yourself.
