The relative of a person who died as a hospital in-patient; the hospital doctor completes the rest.

How to fill it in
- Download the form and print it. Fill it in by hand, in clear block letters, with a blue or black pen.
- Fill in the blanks the form prints: Name of the deceased, Son / wife / daughter of, Resident of.
- Where it goes: Give it to the hospital doctor to complete; the form is sent to the Registrar, Births & Deaths, MC Chandigarh, along with the Death Report (Form No. 2). The portal does not name a counter.
- Official time limit: 7 days, as listed on the MC portal for issue of a death certificate.
- Sign where the form asks you to, check that no blank is left empty, and keep a photocopy for yourself.
