How to fill it in
- Download the form and print it. Fill it in by hand, in clear block letters, with a blue or black pen.
- Fill in the blanks the form prints: Date of still birth, Sex, Father's name, Father's UID number, if any, Mother's name, Mother's UID number, if any and the others.
- Where it goes: The department does not publish where this form is handed in. Ask at the Births & Deaths branch, MC Chandigarh.
- Sign where the form asks you to, check that no blank is left empty, and keep a photocopy for yourself.
