PCL-LT-1: application and agreement for an LT electricity connection, load change or change of name

How to fill it in

1. Download the form and print it. Fill it in by hand, in clear block letters, with a blue or black pen.
2. Stick a self-attested passport-size photograph of the applicant or authorised signatory in the box at the top right. Leave item 1 (office details) for PSPCL.
3. Items 2 to 4: say what you are applying for (new connection, extension of load, reduction of load, change of name or reconnection), the category of supply, and whether the supply is permanent or temporary (if temporary, the period from and to).
4. Item 5, applicant particulars: type of applicant, name of the applicant, name and designation of the authorised signatory if any, the identity proof and the proof of ownership or occupation (self-attested copies to be enclosed), and your category if you belong to one the form lists (SC, ST, BC, BPL and so on).
5. Still in item 5, give the premises where the connection is needed (plot, house or flat number, sector or colony or street, village or town or city, landmark, PIN) and your communication details (mobile number, e-mail ID, Aadhaar number).
6. Item 6, applied load: the connected load (kW), contracted load (kW), contract demand (kVA, for applicants of more than 20 kW) and the purpose of supply.
7. Item 7: list any connection you already hold on the same premises (account holder, account number, address, category, connected load, contract demand). If you are applying for an extension of load, also fill the table of load applied, existing sanctioned load and the totals. Item 8: answer Yes or No on installing your own approved meter.
8. Item 9 is the declaration. Read it, then sign as applicant and fill the date and place. Where it goes: ask at your local PSPCL office where this form is handed in, and which papers it wants with it. PSPCL does not print that on the form page.
9. Check that no blank that applies to you is empty, and keep a photocopy for yourself.
