PCL-LT (DS 20 kW) 1A: application and agreement for a home connection up to 20 kW

How to fill it in

1. Download the form and print it. Fill it in by hand, in clear block letters, with a blue or black pen.
2. Stick a self-attested passport-size photograph of the applicant in the box at the top right. The form is printed in Punjabi and English.
3. Item 1: write the name of the applicant and the Aadhaar card number. A self-attested copy of the identity proof is to be submitted.
4. Item 2, address: house or premises number, street number, area or colony, city or village, telephone number, mobile number and e-mail address (in English).
5. Item 3, category of supply, is printed as Domestic Supply (Residential Only). Item 4 is the schedule of electricity-consuming apparatus: fill the number and the wattage of each for light points, fan points, wall sockets, power plugs, ACs and other or motive loads, then the total kW.
6. Item 5: if you already have a load, give the existing load or demand and the account number, then the additional load or demand required and the total. Then go on through the remaining numbered items on the form in order.
7. Sign at "Signature of the applicant" after reading the declaration. Leave the boxes for the PSPCL officer, the verifying official and the load sanctioning authority to PSPCL. Where it goes: ask at your local PSPCL office where this form is handed in, and which papers it wants with it. PSPCL does not print that on the form page.
8. Sign where the form asks you to, check that no blank is left empty, and keep a photocopy for yourself.
