Application for a border area certificate (Punjab)

A resident of a Punjab village or town in the border area who needs a border area certificate from the Revenue Department.

Issued by: Government of Punjab (Sewa Kendra services)

The form itself is in English, exactly as the issuing office prints it.

How to fill it in

  1. Download the form and print it. Fill it in by hand, in clear block letters, with a blue or black pen.
  2. Fill in Part 1 in order: who is filling the form and the relation to the beneficiary, then the beneficiary's name, gender, date of birth, place of birth, father's and mother's names, address with PIN code and district, marital status, contact number, voter ID and Aadhaar. Stick a self-attested photograph where the form shows.
  3. In Part 2, say how the service is to be delivered (Sewa Kendra or by post) and the office that will process the application.
  4. Sign the declaration with the Aadhaar statement, then fill in the self-declaration page (your name, age, address and district) and sign it with a self-attested photograph.
  5. The form also carries the Sarpanch / M.C. report, the halqa Patwari / Girdawar report, and the Sub-Divisional Magistrate's remark. These are filled by those officials, not by you. Leave them blank.
  6. The last page is a letter of request to the Sub-Divisional Magistrate. Fill in your name, parent's name, village and tehsil, sign it, and submit the set as the Punjab government directs.
