Subject: Death of pensioner Harpreet Singh, PPO no. PPO/2016/12345 — application for family pension

Sir or Madam,

1. 1. The pensioner. Harpreet Singh drew pension as a Government of Punjab or Chandigarh Administration employee, under that government's pension rules. The pensioner last served at: Department of Education, Punjab; retired 30 June 2016. The Pension Payment Order number is PPO/2016/12345. The pension was paid through Punjab National Bank, Sector 17, Chandigarh.

1. 2. The death. Harpreet Singh died on 1 April 2026 at Chandigarh. The death certificate is DTH/2026/118, and a copy is enclosed. Please record the death against the pension from that date.

1. 3. My claim. My relationship to the pensioner: Spouse (widow or widower) of the pensioner. How I depended on the pensioner: Unmarried daughter, living with and supported by the pensioner. I apply for family pension under the pension rules that applied to the pensioner.

1. 4. The rest of the family. The other members of the family who may claim are: Son Aarav Singh, 28, Sector 35, Chandigarh; daughter Meera Singh, 25, Ludhiana. If none, write None.

1. 5. What I ask you to do.

(a) Stop the pensioner's pension from the date of death, and tell me in writing whether any amount paid after that date has to be returned and how.

(b) Start family pension in my favour from the date the rules allow, and pay me the arrears from that date.

(c) Tell me in writing the rate at which family pension is payable to me, how it is worked out, and from what date.

(d) Tell me in writing the claim form and every paper you need from me and from the other members of the family, and issue a fresh Pension Payment Order or authority for my pension.

(e) Pay to me any other money that falls due to the family on the death of the pensioner under the rules, and tell me what it is and how it is worked out.

(f) Pay the amount due to the account described in paragraph 6.

1. 6. Where to pay. Please pay the amount to the account held by Naina Singh with State Bank of India, Sector 35, Chandigarh, account ending 4821. I will give the full account number on your own form and not in this letter.

1. 7. What I state. I have given the facts above as I know them. I will tell you at once if any fact changes, including if I marry again or become entitled to another pension, where the rules ask me to tell you.

1. 8. If the application is not decided. If you reject the application or do not reply within the time allowed, I will write to the head of your office and then to the pension grievance officer of the department concerned, and I am free to go to the court or tribunal that hears pension matters for the pensioner's service.
