A birth was not registered within the twenty-one days the Registration of Births and Deaths Act, 1969 ordinarily allows, and a delayed registration application needs a sworn affidavit to go with it — the Act itself requires one once the delay passes thirty days. Between thirty days and one year after the birth, this affidavit accompanies an application for the Registrar's written permission to register it, together with the prescribed fee.

Beyond one year, it accompanies an application to a Magistrate of the First Class, who alone can order registration at that point, after verifying the birth. Confirm which of the two bands applies to your dates before filling this in, and expect the Registrar or the Magistrate to ask for independent proof of the birth — a hospital record, a doctor's or midwife's certificate, an immunisation or Anganwadi record — alongside this affidavit, not in place of it. A late-registered birth certificate is frequently asked for at school admission, for a passport, or for a government scheme, so check what that particular use needs before relying on this affidavit alone. Not for a birth you can still register within the ordinary twenty-one days, or within thirty days on payment of a late fee alone — neither needs this affidavit.
